The Chapter/ Coordinator/ Chaperone Registration Form contains a checklist for all required items to be sent to National for your to team to participate. This document also contains the forms for the chapter president, coordinator and chaperone(s). All forms should be read and signed. Dress Code Policy and the Penalties and Infractions Policy information are found in the appendix. The information in the Appendix should be kept for your reference.
There are separate documents for students. (The student.doc file and a publication waiver form should be read and signed by the parent or guardian and the student.)

THE FOLLOWING SHOULD BE SENT IN:
1 – Checklist complete with names (page 2)
2 – Chapter form (page2/3)
3 – Coordinator form (page 4/5)
4 – Chaperone forms (page 6/7 and/or 7/8)
5 – Student form (from student form doc, pages 2-8 ) copy of medical card and student id
6 – Publication Release for coordinator, chaperones and students
